The following tools can be helpful for ACA purposes. While some of these tools come standard with Avionté, others are available as part of the ACA Package. For more information on the ACA Package, contact your Avionté Representative

1095 C Report

The IRS version of form 1095C. This is the Employer-Provided Health Insurance Offer and Coverage Insurance Form. This form is required to be distributed to employees of an ALE member who is full-time for one or more months during the calendar year. SSN, Year, Branch, Primary Order By (Last Name, First Name, SSN), Primary Mailing Address (Primary Address, Home Address, W2 Address, Previous Address), Include (All 1095C, Only Corrected 1095C), Sent to the IRS (True, False) Populated Federal 1095C Forms Back Office This report makes printing and sending 1095C Forms easy!

1095 C No Instruction Page

The IRS version of form 1095C - without having to print page two, the instruction page. This is the Employer-Provided Health Insurance Offer and Coverage Insurance Form. This form is required to be distributed to employees of an ALE member who is full-time for one or more months during the calendar year. 1095 C Report 2015

Get Employee 1095-C Info AQ

Gives details of employee 1095-C in an AQ or spreadsheet format   Populated information that appears on employee's 1095-C form. Back Office This AQ allows you to preview the information that will be on employees' 1095-C forms in a mass way.

1095 C Label

This creates formatted mailing labels for envelopes to mail the 1095-C form to employees. You can print this report directly onto 5160 label paper Entity Type (Agency, Contact, Customer, Employee). Status, Contact Method, Address Type (Home, Previous, Secondary), Entity Name, Branch, Start Date, End Date, Exclude Last Check/Invoice Date (True, False), Show Contact Method (True, False) Populated information appears on the label.  Front Office, Back Office This report makes label-making easy. Use 5160 label paper when printing. 

1095 C Label Report 2015

This creates formatted mailing labels for envelopes to mail the 1095-C form to 2015 employees. Entity Type (Agency, Contact, Customer, Employee). Imports and Exports

Pulls employee deductions into the system for 1094/1095 reporting. Supplier, As of Date, Measurement Period Mapping Group, Processing Method, File, Year ID, Check for Duplicate SSN Back Office The Standard Benefit Import is a standard report that includes all employee deductions during 1094/1095 reporting.
Essential Staff Care (ESC) MEC Benefit Import* Imports data for employees who use the Essential StaffCARE Minimum Essential Coverage (ESC MEC). Supplier, As of Date, Measurement Period Batch Name, Description, Batch Status, Mapping Group, Processing method, File Back Office This import marks employees who use the ESC MEC plan.

Essential StaffCare (ESC) FI-MVP Bronze Plan Export*

Exports data for employees who use the Essential StaffCARE (ESC) FI-MVP Bronze. Supplier, As of Date, Measurement Period Batch Name, Description, Batch Status, Mapping Group, Processing method Back Office This export shows the employees who use the ESC FI-MVP Bronze Plan.

ACA Export - IBF*

Exports data for employees who use IBF. Supplier, As of Date, Measurement Period Batch Name, Description, Batch Status, Mapping Group, Processing method Back Office This export shows the employees who use IBF. 

ACA Export - Nelco*

Exports data for employees who use Nelco. Supplier, As of Date, Measurement Period Batch Name, Description, Batch Status, Mapping Group, Processing method Back Office  This export shows the employees who use Nelco.

*Indicates resources deployed at client request. Create a utility ticket in the Support Center to add the selected imports and exports.

 

 

Standard Reports

Each of these fields is broken down to specific values. Back Office This report is a useful resource for payroll processing or for determining the cost of running the business for a specific time period.
Check Register Shows paycheck information for all employees Date Type (Check Date, Accounting Period Date), SSN, Start Date, End Date, Branch, Employee Type (All, W2, 1099), Sort By (Employee Name, Check Date) Employee Name, Check #, Check Date, Gross Amount, Total Taxes, Total Deductions, Net Check, Is DD?. Further broken down: Transaction (Type, Units, Rate, Total Pay), Tax (Tax Name, Taxable Gross, Tax Amount), Deduction (Deduction Type, Amount), Benefit (Type, Amounts, YTD Benefit), Accruals (Plan, Accrual Units, Balance, Depleted Units.)

ACA Companion Census AQ

Shows an employee's total hours and ACA Eligibility as well as total wages, benefits, and benefit amounts.

Employee ID, First Name, Middle Name, Last Name, SSN, Employee Status, Employee Branch, Birth Date, Original Hire Date, Recent Hire Date, First Check Date, Recent Check Date, Effective Insurance Date, Assignment Max End Date, Street 1, Street 2, City, State, Zip Code, Email Address, Cell Phone #, Gender, Total Wages, Total Hours, Deduction Amount, Benefit Amount, Is ACA Eligible, Current Stability Period End Date, Measurement Start Date, Measurement End Date, Measured Type, Measurement Total Hours, Measured Eligibility, Reason for Deactivation, On Assignment Back Office This report is useful for tracking healthcare eligibility as well as other benefit participation. ACA Companion Insurance Offers AQ

ACA Companion Employee Offers AQ

Displays a list of all offers made in the ACA Companion Application. Audit AQ

Lists changes to specific tables within the application Start Date (Audit Date Modified), End Date (Audit Date Modified), User Name, Table name, Column Name Audit Type, Table Name, Column Name, Old Value, New Value, Date Modified, User Name, User Description Front Office, Admin This report lists all changes made to any table along with the value prior to the change, the date the change was made, and the User that made the change.

Employee Deduction/Contribution List AQ

Employee Pay History page Displays all of the payments and other billing associated with past paychecks for a selected employee. N/A Branch, Check Date, Check Deliver, Check Number, Date Cleared, Deductions, Direct Deposit, Employer Cont, Gross Pay, Name, Net Amount, See Paycheck, Select, Status, Supplier, Taxes, W2, Year ID, YTD Gross Back Office This tool records all information pertaining to an employee's paycheck history. Employee ACA Eligibility page Displays if an employee is eligible for ACA.  N/A No (not eligible), Yes (eligible), Measured Start Date, Measured End Date, Measured Type, Total Hours, Measured Eligibility Back Office This tool allows you to view and edit an employee's ACA eligibility status.
